For a smoother and quicker enrollment experience for your student, please use our online enrollment system. We understand the convenience of traditional methods, so paper forms remain available for those who prefer them.

To Enroll a New Student With Paper Forms:

  1. Complete the Student Enrollment Forms Part I and Part II (also available in SpanishFarsi, German, Arabic).
  2. Select one (1) of the following affidavits below:
    1. If you own/rent your home and live with the student, use the Affidavit of Enrollment and Residency.
    2. If someone else owns/rents your residence and you live there with the student, use the Affidavit of Enrollment and Residency When Living With District Resident.
    3. If the student lives with you under non-parental custody, use the Affidavit of Non-Parent’s Custody, Control, and Responsibility of a Student.
    4. Note: All affidavits must be notarized.  A Notary is available at the District Office by appointment.
  3. Assemble the following supporting documents:
    1. Proof of residency documentation.
    2. Student’s birth certificate.
    3. Required medical forms.
    4. If the student is receiving special services, a copy of the current plan.
    5. For Batavia High School students, an unofficial transcript or 8th grade report card. An appointment with a guidance counselor will be arranged for course selections.
  4. Submit forms and documents:
    1. Email scans/photos to enrollment@bps101.net (redact sensitive info).
